In PFFS too, you don’t have to choose any primary doctor. You can go to any hospital or doctor where PFFS is accepted.

Medicare special needs (MSN) plans are suitable if you have medical conditions that need expensive and continuous care and attention. People with chronic diseases and poor health status go for this plan to save money.  

The premium you have to pay for Medicare advantage plans purely depends on the plan you choose. If you go for plans with maximum coverage, you have to pay more money as premium.
